Pre-Appointment Checklist
- Click here to fill out the online paperwork. This must be completed 24 hours prior to your appointment. If you cannot access the online paperwork, download and print.
- Bring your medical and Vision Insurance card(s)
- Bring all the glasses you use, including your prescription sunglasses
- If you wear contact lenses, wear your contacts to the appointment
Arrive Time
- Arrive 10 minutes early to your appointment.
